La preeclampsia y laeclampsia posparto podrían surgir días o semanas después de unnacimiento normal, según informan investigadores en la revistaObstetrics & Gynecology.
La eclampsia es una condición relacionada con el embarazoque se presenta después de una preeclampsia, una complicacióngrave de la gestación caracterizada por hipertensión arterial yexcesivo y rápido aumento de peso.
Con una mejor atención prenatal y uso profiláctico desulfato de magnesio en el período perinatal, se registró unareducción marcada de la incidencia de preeclampsia en losúltimos años, y un cambio hacia una mayor frecuencia deeclampsia hacia el período posparto tardío.
El equipo del doctor Zain Al-Safi, de la Escuela deMedicina de la Wayne State University y del Centro Médico deDetroit, evaluó la demografía, el tratamiento clínico ycomplicaciones de la preeclampsia posparto demorada en 152mujeres.
Las pacientes habían sido reingresadas al hospital entredos días y seis semanas después del alta inicial con undiagnóstico de preeclampsia posparto.
Casi dos tercios de las mujeres carecían de antecedentes dediagnóstico de hipersensibilidad a la enfermedad en el embarazoactual. Veintisiete habían sido tratadas con sulfato demagnesio durante el período perinatal.
El 69 por ciento de las mujeres se presentaron con dolor decabeza. Otras tenían síntomas como falta de aire, visiónnublada, náuseas, vómitos, edema, convulsiones, otros déficitneurológicos y dolor epigástrico.
El 15 por ciento de las pacientes desarrolló eclampsia, ymás del 90 por ciento de los casos se presentaron dentro de lossiete días del alta del hospital.
En un modelo logístico regresivo, la edad joven se asociócon un 13 por ciento de aumento en las posibilidades dedesarrollar eclampsia, mientras que una readmisión con bajahemoglobina se vinculó con un 25 por ciento menos posibilidadesde avance a eclampsia.
Sólo 28 de 543 mujeres diagnosticadas con preeclampsiasuperpuesta a hipertensión crónica durante el período deestudio reingresaron al hospital por preeclampsia posparto, yen ninguna de ellas el cuadro avanzó a eclampsia.
"Debería hablarse de la posibilidad de preeclampsia yeclampsia posparto demorada luego del nacimiento del bebé, yasea que la paciente desarrolle o no enfermedad hipertensivaantes del alta hospitalaria", concluyeron los investigadores.
"Las pacientes deberían ser informadas de que podrían estaren riesgo de preeclampsia o eclampsia hasta unas seis semanasdespués del parto", añadió el equipo.
