Los pacientes deberían comenzar a tomar los medicamentos contra el virus del sida más temprano para tener más opciones de sobrevivir, dijeron el jueves investigadores.
En un análisis realizado a más de 45.000 personas con VIH en Europa y Norte América se descubrió que los pacientes eran un 28 por ciento más propensos a desarrollar sida o a morir si postergan el tratamiento hasta el punto actualmente recomendado en muchos países.
No hay cura para el virus de inmunodeficiencia humana (VIH) que causa el sida, pero la combinación de medicamentos puede impedir que el virus se duplique y dañe el sistema inmunológico.
Los médicos normalmente no inician el tratamiento hasta que haya pruebas de daño en el sistema, medido a través del recuento de las células inmunológicas llamadas CD4.
Las actuales pautas piden el tratamiento sólo después de que el recuento de las CD4 sea menor a 350 células por mililitro de sangre.
Jonathan Sterne, de la Universidad de Bristol, en Reino Unido, y sus colegas descubrieron que esperar hasta que el recuento de las CD4 hayan caído hasta 251-350 estaba asociado con peores resultados que los obtenidos al comenzar la terapia en el rango de 351-450.
El equipo -cuyos hallazgos fueron publicados en Internet por la revista Lancet- concluyó que un recuento de 350 células debería ser el umbral mínimo para comenzar el tratamiento.
Decidir cuándo comenzar a tomar los medicamentos tradicionalmente ha sido visto como un acto de equilibrio, debido a que las fuertes medicinas pueden tener graves efectos secundarios y también se puede desarrollar resistencia.
Los investigadores dijeron que ahora es más fácil resolver estos problemas debido a la llegada de un rango más amplio de medicamentos, y los médicos deberían enfocarse en llevar a los pacientes antes a terapia.
"Es importante que las personas con posible riesgo de tener VIH sean evaluadas regularmente, así que si resulta que están infectadas pueden recibir el cuidado y tratamiento necesario", dijeron.
Alrededor de 33 millones de personas en el mundo están infectadas con el virus del sida, y la mayoría de ellos viven en África y otros países en desarrollo.
